A digital platform called PM-Family Care Tracker (PM-FCT) has been introduced in Gujarat’s Gandhinagar Lok Sabha constituency to consolidate health, nutrition, and education information from pregnancy to adolescence. This initiative, overseen by Union Home and Cooperation Minister Amit Shah, aims to enhance the monitoring of maternal and child welfare outcomes through an integrated system. The platform will track beneficiaries from pregnancy to 18 years of age, including pregnant women, lactating mothers, and children, to improve coordination among health, education, and nutrition services.
The PM-FCT system will integrate data from various welfare programs like Integrated Child Development Services (ICDS), Reproductive and Child Health (RCH) program, Poshan Abhiyaan, and school health initiatives. Currently, different departments in Gujarat maintain separate beneficiary identification systems. The new platform proposes linking these datasets through a common unique reference, such as Birth Registration Number (BRN) or ABHA ID, to facilitate data sharing across systems.
Officials anticipate that this integration will enable continuous monitoring of health, education, and nutrition indicators throughout a child’s developmental stages. The platform will create a comprehensive profile for each beneficiary, allowing monitoring of key parameters like immunization status, anaemia, body mass index, and birth weight. It will also track educational progress, including transitions to formal schooling, school retention rates, and adolescent health indicators.
